As legal guardian of the above-mentioned participant, I consent to his/her participating in the programs, activities, and games offered by MAS Basketball Training, LLC (“MAS”). I also consent to being a part of any leagues of which MAS may be a member or with which they may be associated. All programs, activities, games, and leagues are collectively referred to as “events” herein. I am aware that participation in any MAS event has some inherent risks and injury can occur. I understand that MAS cannot list every possible injury that can occur, and I hereby acknowledge that the number and types of injuries are numerous. On rare occasions these injuries can be serious, including traumatic brain injury, paralysis, or even death.

I hereby agree that I am assuming the risk and assuming the risk for my child by allowing his/her participation in MAS events. In consideration for allowing my child to participate in MAS events, I forever release, agree to hold harmless, and indemnify MAS Basketball Training, LLC, it’s directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasees of the premises used to conduct the event (“Releasees”) from any and all liability for any and all damages and injuries suffered by my child while under the instruction, supervision, or control of any of the above mentioned parties. I agree not to bring legal action against the releasees for any damages or injuries claimed to have been suffered by my child while under the instruction, supervision, or control of any of the above-mentioned parties.

In the event that an injury occurs, I authorize MAS staff to administer first aid to my child and/or call 911 to take all proper action and use the emergency service available at the nearest hospital if necessary. In case of an extreme emergency, I authorize the emergency personnel to take proper action

I attest that I have the legal binding authority, apparent and actual, to enter into this agreement and bind myself, my spouse, and my child to the terms of this agreement.

This release of liability and waiver, having been read thoroughly and understood completely, is agreed to in content and intent and I understand that I am giving up substantial rights and am doing so voluntarily, knowingly, intelligently and without any inducement or promises in return.
